Surely that is determined by certificates and legal documentation like passports. If the passport says Ms then they attend the female events/clinics. I would assume all medical records would be kept should they be needed, but once fully changed they are in all tense and purposes female and should be treated as such.
For me it isn't a matter of keeping secrets of the past, it is more concerns for the future. ie, Most people assume to have a family and I would assume that babies may be problematic in the traditional sense, so serious conversations would need to be made, but at what stage do you talk about stuff like that?
It's the same with Escorting. It is my right not to tell anyone I go on a date with about my work, but if things move on, then there comes a point where I would feel compelled to say something before allowing deep attachment. That is my personal view though and may not be the way everyone feels. Why does the whole world need to know everything about you?
