Most of what I like falls under GFE so would rarely be classed as an extra, I’m not into ticking boxes during appointments anyway, I like them to flow naturally so having to arrange something in advance would mess with the vibe, in my head anyway.
I tip occasionally, I show my appreciation in lots of other ways as well even though it’s never expected.
