The risk of transmission of HIV by all means (unprotected sexual intercourse, blood borne, mother to fetus) is actually a lot less than you would think, lets say in comparison to Hepatitis B. There are other factors involved such as the viral load in the person with HIV i.e. the higher it is the more likely the individual will transmit it to others when having unprotected sexual intercourse. Of course you won't know this at the time. So it just makes common safe sense to always have protected sexual intercourse. I agree with one of the other posters who said that the risk of getting HIV is just as high or higher from having unprotected sex with any non escort partner you may have if neither of you has checked your status.
Back to the OP's question about OWO and sexual trasmission the risk is very low and not possible to estimate. Factors that will increase your risk of getting HIV during OWO or reverse oral would be if either party has active genital ulcers or abrsions (for herpes) at the time, or in fact mouth ulcers.
Again all of this is a question of risk assessment and everyone has to make up their own mind. "Safe sex" is a myth. There is no such thing. There are just degrees of safety. The common ground most people would agree on is "protected sexual intercourse" with an unknown partner should be the rule.
Anyone of us involved in this industry (client or escort) should get ourselves checked out regularly (e.g. 3 monthly - HIV/ Hep B/ and other treatable bacterial infections like gonorrhoea, chalmydia and sphylis).
I suspects escorts take this more seriously and are more responsible about this than a lot of clients in general.
