I would have more faith in your survey if you had managed to spell the title properly. Decision is the correct term. Please may I ask what your doctorate is in and also your reasons for cut and pasting those questions here?

My Doctorate is in Cognitive science. I cut and pasted those questions for my own research.
Now this isnt a fake, I have permission off the managers here. I have forwarded them my details and they have checked them out.
