The fact is that you can certainly contract STDs (including the most serious ones) from uncovered oral sex. This is a risk we are all taking and there is no safety net currently in place so you are right to be cautious.
At the same time the chances of contracting an STD through OWO are slim by my calculations as I think most escorts would be protective of their own health to begin with.
