    my only cocern is if you have diabetes if so see your doc, if not over the counter treaments should suffice.

    But you still have athlete's foot. Get some Hydrocortisyl or Daktarin cream. Even if it's not athlete's foot, which it probably, it is more that likely some form of fungus and the cream should clear it up. If you leave it and keep scratching it to the point where it bleeds you will eventually get an infection in it which will require antibiotics to clear up. Take the simple option, start with this and see how you get on. What's the worst that can happen, apart from me being right???

    Always take off your socks last when undressing and put them on first when dressing. This may stop it spreading to your stick and giggle bag area while you are treating it.
