If you got this "quiz" from another site, or forum, I would advise not answering anything on that page. It looks like an attempt at Social Engineering to work out people's date of birth, gradually. As it is often a security question for online accounts.
Every time you see something like this, or "you pornstar name is your first Pet's name and your mother's maiden name" stay away from it.
