Many never use the PM. system, even if lt's listed on their profile.
Of those that do, your last sentence is probably correct. They may also respond to someone they don't know , if that person has a presence here, eg reviews. They can then get an idea of what kind of person you may be and maybe feel that you are more likely to turn up for an appointment.
The most popular escorts will get lots and lots of PMs and many of these will just be people they don't know making conversation or even being obnoxious in some cases.
If PM-ing someone (even more so if they don't know you) keep it clear and short and sweet, don't try and start a big long conversation.
