Since escorts don't go bareback in the vast majority of cases, I seriously doubt they'd suck some guy off if theyd recently bitten their tongue til it bled, or if they had some sort of mouth infection. It's as much their risk as the guys.
Yes, it's safe
No, too risky

I've looked for owo for the past 25 years.
I average 3 visits to escorts a week.
Never had an issue. Never caught anything.
I'll continue seeking owo.
Minimal risk

I was quite surprised when I realised that most top escorts, so careful in everything else, advertised OWO, but I now understand that it is so popular with clients that it would significantly affect business if they didn't.
I also have tried to research it, but it is difficult to find anybody who knows what they are talking about. Most Google hits are on general "Web Doctors" who don't have any proper expertise in this. The best analysis I found is this panel discussion, which focused on HIV risk.

It is very long and detailed, but the main points are:
- There is almost no risk to the person receiving OWO
- OWO is much less risky than unprotected anal
- OWO without CIM is agreed to be a very small risk
- OWO with CIM on a partner with unknown HIV status has a chance of 1/2500 of transmission
- About 5% to 15% of HIV transmission is estimated to be through oral sex
That statistic of 1/2500 sounds low, but the risks increase quickly if you allow CIM many times. Using those numbers, if you allowed CIM 250 times by men of unknown status, the risk of contracting HIV would be around 1/10. The statistic came from a study of gay men in San Francisco, where someone with unknown HIV status is probably much more likely to actually have HIV than a random EI client but it should still give you pause for thought if you allow CIM.
There are more sti than hiv.... hepatitis would be right up there with one of my fears, no matter how clean and shiny your cock is.... you will never see a virus. You just need a small cut in your mouth for transference. People damage their tender gums eating lunch.... a crusty roll could cut you. My opinion is that it's a risk too high....i know too many doctors. One's a client and my god he's over the top with hygiene and safety.... doubling up condoms and using clinical gels on me and him....i think he's a bit ocd though. But my mate's have advised me not to and one of them works as a gp in a clinic that specialises in sti's, she does all my tests. It limits my business with the punters on the boards... but most of my clients don't even know the boards exist, so i don't feel pressured. some come to see me for the reason i don't do certain things in my fave's. It's entirely up to the individual be they escort or punter. Just my opinion.

The safest form of sex is no sex at all
Any kind of sexual practice carry some risk, from really low to major.
Even really low risk carrying activities could change into big risk activities if anything happens in between. A little nail scratch, some lipstick on the condom that could cause tiny holes in it, touching condoms with oily hands etc.
Heck, even sitting on some toilet could give you something if unluckly.
So, my advice, for everyone, just do what makes you happy and if you feel the risk is too big don't take it.
If you are someone that's always prone to get sick and get infection just stick to one regular partner or simply masturbate, you're very likely to catch something at some stage if you do otherwise. If your imune system is strong (like mine), use your cop on for anything you do. Information about how STDs works and spread, wise decisions regarding partners and activities, together with a lot of doctor checks are usually working best for most people.
