Whoever invented that saying has never heard of a pan-proctocolectomy, the surgical removal of the large bowel, rectum & anus, the hole where the anus was, is sown up thus the asshole is gone. Taken it's place is a stoma (surgical attachment of the small bowel to the outside of the abdomen).
