I am being perfectly serious when I ask.. Do mothers not show their sons how to keep themselves clean from when they are young? By clean, I mean showering or washing every day including pulling back the foreskin and washing it!
One client who asked me what the biggest problem I found with the job, and expressed his horror when I told him it was having to deal with unhygienic clients.. He was one of the worst I had ever met. Obviously didn't realise I was giving him a hint!
I spent a whole weekend with him. Not once did he go in the shower, shave, have a wash, or brush his teeth..not once! despite my telling him there was lots of hot water and why didn't he go and enjoy a nice hot shower. How absolutely disgusting.
A lady once told me that when a client protested as she got out a condom for oral, despite him having showered, she wiped the cheese from under his foreskin and shoved it under his nose, asking him, would you put that in YOUR mouth? Good for her I say!
I could go on and on....
I never did find a way around the discomfort of telling a client that he is not clean. Point here is, why should I have to?
Back to the OP's point. No, we are not under any obligation to provide any intimate services, not with anyone, not ever.
